I'd like to refer you to Eileen Chcichin at the Jewish Home and Hospital in NYC or Mary Ersek at Swedish Hospital in Seattle. Both have extensive experience and many contacts. In addition there are a lot of resources on our website:

I would also suggest talking to Dr. David Weissman, a CAPC consultant and a physician-educator at Wisconsin Medical College. He was the PI on a project designed to assist skilled nursing facilities in creating action plans to initiate pain management programs. (For a review of the study, see: Weissman, D.E., Griffie, J., Muchka, S, & Matson, S. (2000). Building an institutional commitment to pain management in long-term care facilities. Journal of Pain and Symptom Management, 20(1), 35-43.) I'm guessing he could probably point you to institutions that were successful in getting a program up an running.
In addition, his group published some materials to help SNFs interested in setting up a program. See Weissman, D.E., Griffie, J., Muchka, S., & Matson, S. (1998). Improving Pain Management in Long-Term Care Settings: a resource guide for institutional change. Medical College of Wisconsin. We have a palliative care consult service that serves the LTC agencies in our community. It is primarily an ARNP practice. jbraun
