Amber,
My hospital is looking at providing Palliative Care consultant services to our local ECF. I am emplyed as Palliative Care NP at our hospital. I need help with billing issues in long term care-is it the same as billing Med part B in the hospital? Do you have any resources for contracts in an ECF and is it best to have the ECF Medical Director as my collaborative physician? Does CAPC have any resources to help establish ECF palliative Care programs?

Amber, who is part of your team for palliative care at the SNF? How do you use the team members? We have an established palliative care consult service at our local hospital. We want to branch out to the SNF. Our hospice already has good relationship with many of the SNF. I am just stuck about how to use the team members (RN, SW, etc.) effectively. Thanks.
