I would also suggest talking to Dr. David Weissman, a CAPC consultant and a physician-educator at Wisconsin Medical College. He was the PI on a project designed to assist skilled nursing facilities in creating action plans to initiate pain management programs. (For a review of the study, see: Weissman, D.E., Griffie, J., Muchka, S, & Matson, S. (2000). Building an institutional commitment to pain management in long-term care facilities. Journal of Pain and Symptom Management, 20(1), 35-43.) I'm guessing he could probably point you to institutions that were successful in getting a program up an running.

In addition, his group published some materials to help SNFs interested in setting up a program. See Weissman, D.E., Griffie, J., Muchka, S., & Matson, S. (1998). Improving Pain Management in Long-Term Care Settings: a resource guide for institutional change. Medical College of Wisconsin.
