There seems to be some disagreement on the definition of Fast stage 7A with many stating that if a patient speaks more than six words in a day but they "don't make sense" that the criteria is met. I would argue that the original Fast paper defines the speech as intelligible words, meaning inappropriate speech would not qualift as 7A. Can anyone state specifically how CMS or others define FAST 7A regarding speech?

There are four fiscal intermediaries (FI) that manage hospice benefits; each has its own specifics regarding eligibility criteria including variable definition of the FAST scoring system. I suggest you find out which FI manages your state and contact them for clarification.

Just to provide feedback, I contacted Palmetto by email and received a ridiculously ill informed response. I am convinced that no one has an answer to this question. Frankly, they should drop the fast score from the criteria. Until then, who knows?
