Post contracts with long term care facilities

I am in the process of beginning palliative care consults within the outpatient settings of SNF/ALF, etc. and looking for input on how to develop the appropriate legal contracts with these facilities?

We are a not-for-profit Hospice & Palliative Care that functions independently but within a larger hospital system. The facilities are not within this system.

Beyond that, I'd urge you to contact a health-law attorney who can best meet your need.
