I don't have a copy of a contract for non-hospice PC in LTC but I would encourage you to explore the resources on the CAPC Website. In the file "Palliative Care and Hospice Care Across the Continuum" is a "Long Term Care" file. In that file is a presentation entitled "Hospice and Non-Hospice Models of Palliative Care Delivery in Long Term Care." This presentation has lots of valuable resources within. The Hospice and Palliative Care Center of the Bluegrass has a non-hospice PC program in LTC and has also developed some educational resources for LTC PC program development. You could contact them directly. Their site is noted also on the CAPC website.
