Hi,

Networking with others doing palliative care in nursing homes is key.

In the meantime, you might look at a paper in the Perspectives on Care at the Close of Life section in JAMA, by a national experts in the field.

It's
Meeting Palliative Care Needs in Post–Acute Care Settings: "To Help Them Live Until They Die"
Laura C. Hanson; Mary Ersek

and available at http://jama.ama-assn.org/cgi/collection/perspectives?page=3

best,
Mike Rabow, MD
Associate Director, UCSF Palliative Care Leadership Center
