Our program has been doing outreach to a local nursing home for months, with variable success. I would appreciate sharing ideas with you. Not sure how this works, so I'll write my email here: frainone@lourdes.com

I too would be interested in sharing ideas and materials. I'm the Chaplain in a 96-bed Long Term Care Home and the Chair of our Palliative Care Committee. We're proud of the palliative care we provide here and families tell us repeatedly that our care was outstanding. My work email is I'd be happy to send some of the materials we use and to see other homes'.
