We have developed several communication tools at our hospitalist programs at two Boston-area hospitals. The the tools are similar to email and SMS, and were designed specifically for communication during care transitions.
We are currently offering the tools free to any groups. we are hoping they can become part of quality improvement and cost reduction programs.
