Post "POLST" form

Approximately eight years ago the state of Georgia had a bright pink, heavy paper, form that was the "POLST" form. It was put out on a trial bases. I do not recall what the acronym stood for, but I did put this into use in our facility with the physicians permission and incorporated it into our care plans. It worked very well, but when I tried to call the number on the bottom of the sheet the lines had been disconected and I never have seen this again. I was told it began in the state of Oregon. Any information about this would be appreciated. Thanks Angie
