Does anyone have experience in working with heart transplant and LVAD teams?

The Palliative Care team at the Mayo clinic (Rochester, MN) has reported their experience with the LVAD patients; contact Elise Carey, MD at Mayo.

We have been working with our LVAD and Transplant teams here for a couple of years. Those teams have been in transition for the past few months, so establishing a more defined relationship has been on hold for that time. We still see their patients in consultation on an as needed basis.
