Post Palliative care contracts

We are just getting started with our inpatient palliative care program. We will see the patients in the hospital and then plan to continue to follow patients for palliative care after they leave the hospital. Do patients have to sign a contract with us for this service. If so, does anyone have a contract they are using that you are willing to share? Thanks!

I would imagine that the answer depends on the legal relationship between your program, the hospital and the outpatient clinic in which you work. I would check with a health care attorney.
