is a copy of one the sheets that we have available to our families. There is a folder with info in each room and these sheets are also available in the living room of our inpt. unit. This is one of several including hydration/nutrition and '5 wishes'.

Tim Ford: thanks so much for that very helpful link to the dying process. We've been using something in our nursing home which came from our regional hospice association but this is more detailed and more helpful. I'd like to use it here--do you know to whom I can give credit for this article? Also, if possible, I would also be interested in seeing a copy of the other information you use.
