Hello,
There is an interesting article describing outcomes associated with implementing a structured palliative care intervention in a trauma ICU:

Changing the Culture Around End-of-Life Care in the Trauma
Intensive Care Unit
Anne C. Mosenthal, MD. et al
The Journal of TRAUMA Injury, Infection, and Critical Care

The same group published a paper describing their intervention:

Mosenthal AC, Murphy PA. Interdisciplinary model for palliative
care in the trauma/surgical ICU. Robert Wood Johnson Foundation
demonstration project for improving palliative care in the ICU. Crit
Care Med. 2006;34:S399 –S403

Not sure this gets at the issue of identifying which patients are in greatest need, but it shows one method of ensuring that trauma patients and their families benefited from a palliative care intervention.
