Our agency has an End of Life order set. There are regional and national suggestions as well. In the midwest, The Institute for Clinical Systems Improvement (ICSI) has websites both for an algorithm and order sets for palliative care and hospice (http://www.icsi.org/ and search for topic). Center to Advance Palliative Care (CAPC) also has some suggestions under the Clinical and Quality Management Tools (http://www.capc.org/tools-for-palliative-care-programs/clinical-tools).

At Backus Hospital in Norwich we now have a Comfort Measures Orders Policy and Order Set that is in accessible in the computerized order sets. I you would like a copy please feel free to email me : efracchia@wwbh.org
