Our agency has an End of Life order set. There are regional and national suggestions as well. In the midwest, The Institute for Clinical Systems Improvement (ICSI) has websites both for an algorithm and order sets for palliative care and hospice (http://www.icsi.org/ and search for topic). Center to Advance Palliative Care (CAPC) also has some suggestions under the Clinical and Quality Management Tools (http://www.capc.org/tools-for-palliative-care-programs/clinical-tools).

We are in the process of developing an end-of-life order set perhaps utilizing the POLST concept. I would be appreciative if you could send me a copy also. Thanks.
