Would you be willing to share the screening tool? We're contemplating implementing a similar process, and any thoughts would be welcome. Thanks!

I tried to cut and paste, but was unable. Go to the CAPC home page > Tools > Clinical and Quality managemtnt tools > Standardized consult, progress notes and billing cards > screening and eligibility for Palliative Care consultation tool. Hope this helps Ben
