I also agree that mandated referrals may not be well received. I do round in the units and "suggest" to the nurses and they can then suggest to the physicians. It does require a lot of continuing education on why it is better to get me involved sooner rather then later...

We do not mandate consults, but use the palliative care screening tool on those patients the bedside team finds effective.
We are currently doing a survey on dialysis patients in ICU and which ones are referred to palliative care. It rarely takes the screening tool now that palliative care is more visible.
