If you haven't already looked at it I'd recommend the "Notes from the Field" article in the June 2007 Journal of Palliative Medicine.

Here at VCU we take similar approach in the ED as we do in any other location in the hospital. We do in-service trainings and education in the ED at least annually. Our consults are mostly limited to 8-6 M-F. The ED can admit directly into our PC Unit 24-7 with approval of the PC attending on call. We don't have any trigger criteria for consult or direct admission. We've been building steadily on positive experiences for several years.
