National Quality Forum Framework and Guidelines
Not sure if this is the right forum but would enjoy some thoughts and insight. I have just started a new (March 2011) inpatient palliative care consultation program at our hospital (no outpatient clinic yet). I am collecting a number of metrics but was wondering, if I can select and chose what will be reported, what few measures should I focus on...
I got an email from on of the policy/admin people stating the following:
"One of my responsibilities is to coordinate the Excellus Pay-for-Performance contract. I am in the process of developing the 2011-2012 one now. That prompted me to write and ask if you have any measures that you keep. Just wondering if there was anything that I could put into this contract. No obligation. A team will be meeting in another week to begin brainstorming all the improvement efforts taking place. We try to identify improvement efforts underway that we have confidence they will achieve their goals.
If you think you might have something, I would love to talk to you. Here are some examples of data collected and goals set by Palliative Care Programs:
• Increasing the number of days between consult and death
• Increase the number/rate of palliative care consults per 100/1000 discharges
• Increase the number/rate of referrals per 100/1000 discharges"
I have the opportunity to make the PFP measures what I want, what would be most beneficial for our very new inpatient based program?
If i need to be in a different forum please point me in the right direction.
