Yes, the Mt Carmel team has had one in place for many years. Their complete scorecard is available as part of the PCLC curriculum, http://www.capc.org/palliative-care-leadership-initiative/

Weissman et al have been publishing a series of articles on metrics for PC, in JPM starting in the fall of 2008. Not necessarily a scorecard but at least some consensus on what to include.

A PubMed search turned up one possible published article about a scorecard for long-term care that may be somewhat applicable:
