thank you for your response. do you have a separate form for your social worker or chaplain?

The social workers have their own notes and the chaplains just do a free text note, but title it Spiritual care. I utilize the social workers and chaplains assigned to each unit, they are not an "official" part of palliative care team, so I ask them to signify when they are charting a note in junction with PC. So chaplain may title their note Spiritual Care/Palliative Care, etc...
