We are a 400+ bed hospital with 450-500 consults per year. We staff a full-time physician, full-time nurse coordinator, 0.5 fte social worker, and 0.5 fte chaplain on a consult basis anywhere in the facility, Mon.- Fri. 8:30-5:00.
How is documentation done by team members out there? Our doctor and nurse coordinator and social worker write in the progress notes, but the chaplain only currently documents that she visited in the nursing computerized notes. Does this meet guidelines for interdisciplinary documentation in the chart or is a separate form required for our service?
