This is definitely a challenge. In one of our community hospitals, social work, chaplains and charge nurses triage patients in the ICU who met criteria for considering a family meeting. One of them would ask/call the primary physician, and if the physician agreed, would take on the role of scheduling the meeting at a time that worked best. Proximity of physicians' office to the hospital was also a factor. Another approach would be to reserve a usual time for family meetings that would be available as needed.
