1. Survey of knowledge of what palliative care is?
you need to first decide what information you are seeking a) palliative care clinical care (e.e. opioid equianalgesic doses) or b) survey about the definition and components of palliative care clinical services?
For a) There is a commonly used and well validated test of palliative care knowledge test within the CAPC product: Top Tools (https://campus.capc.org/Toolkit_Contents.pdf).
For b) it is probably best to make your own survey based on how palliative care is positioned at your setting.
2. Survey of when to refer to palliative care. This would depend on your particular setting and how they have marketed their services--although there are many similarities across programs, there are considerable differences as well.
