I agree with Evelyn. Although your post is almost a few years old, I would also be interested in the 3 x 5 card you mentioned, if it is still available.

If anyone else has other "eligibility" teaching ideas to share, I would love to hear of them!

Thank you for your question Lisa. At UAB we have developed "pocket cards" entitled "Got Pages." We provide those cards to all housestaff and other MD's on an ongoing basis. We use it as our calling card for them. On the inside leaflet it has the eligibility criteria for inpatient consultation coupled with an equianalgesic dosing chart which is the hook that makes them sometimes hang on to it.

In addition, we have developed flyers, etc. that explain appropriate patients for outpatient supportive care referral.
