Prior to the new CoPs we used to complete admission paperwork at the hospital for new hospice patients being discharged home and then do a "tuck in" visit when they arrived home. The thinking was that they are sitting around the hospital anxiously awaiting discharge home and it would be convenient to complete the admission then rather than have to bother them too much once they arrived home. When the new CoPs came out in Dec 2008, they said the admission has to be completed where the patient will receive services. If you are going to be providing inpatient services for a couple of days prior to discharge home you could do the admission at the hospital but not if the first day of services is a routine home care day. Regs say that "a nursing assessment" must be completed within 24 hours of signing admission paperwork. Then an initial and complete assessment must be completed within 5 days.
