would like information about the process if you are a hospital-based hospice providing Palliative Care consultations:
Does the hospital have a policy?
Criteria for referral?
Staff time billed to hospice or have separate palliative care cost center?
Are you on-call? How do you handle that with hospice call?
Anything else you can tell me.
Our hospital is 120 bed - our hospice dept. has 5 FT and 1 PT staff - hospice serves 3 counties - we are rural based.

Mount Carmel Health System is a community hospital system with a PC consultation service in three hospitals and a hospice program as a part of the system. The PC service had its origin in hospice but the PC consult team functions out of two cost centers--Palliative Care and Hospice--and has functions in both cost centers. There are administrative policies in the system related to the PC consult service and the acute palliative care unit at each hospital. The criteria for referral are limited and essentially any person who has an advanced disease process and continues to meet criteria for acute hospitalizaton and has needs that could be addressed by the PC service meet criteria. We also use clinical indicators to identify patients who would be appropriate for PC services. The Physicians and APN's who see patients in the hospital bill for their services through the professional billing department of the hospital. We have physicians on-call 24/7 for PC and for hospice.
