Post Hospice GIP orderset for hospital use

we are trying to get an orderset for the hospice MD to use when admitting to a GIP bed in our hospital.Does anyone have an orderset they use that they cold share. Thank you Ellen.

I have seen a couple of different approaches to Hospice GIP ordersets. Some are very detailed and outline multiple symptom algoithms and others are less detailed relying on initiation of a new plan of care for the more acute situation. In the less detailed model an additional page of orders is generated. We also find that instituions are using medication reconciliation forms to indicate medication orders continuation or discontinuation at time of admission. I would encourage you to see what ordering process is in place for the acute care patient and then create the hospice order set with the acute care porcess in mind.
