Our hospital has a policy related to the transfer of a patient from acute care to Hospice inpatient. the doctor can also order "comfort Care" which includes the choice to involve hospice but does not mandate hospice. the comfort care order sheet is identical to the standing orders for Hospice Inpatient. I would be glad to fax you a copy of the policy and the order sheet. Just send me your fax number. Neither one actually assures EOL discussion. We have learned to ask the person calling for hospice referral or "consult" if the family/patient has been told their prognosis by the physician. WE frequently will get a "consult request" of someone that has been in the hospital several days and is due for discharge that day or the next and someone finally had an EOL discussion with the family now that the patient is being discharged. Very frustrating!
