We are a 112 bed non-profit hospital (catholic) that is working to build our Palliative Care Team (we have a Director/LCSW-C and a Physician (.25 FTE's), we are working to get Nursing on board. In our hospital, there was a GIP contract signed with a hospice in 2008. We are seeking guidence around Joint Commission as well as setting up some of the details with converting a hospital patient into a hospice GIP patient. thanks. kimberly.barrett@stagnes.org.
