To create a contract, you will need to be familiar with the provisions of the CMS Conditions of Participaton for inpatient hospice services and with licensure/regulatory provisions for hospice in your State. The National Hospice and Palliative Care Organization has contract templates available through it's Marketplace - which would give you a good place to start. You would, of course, want to tsilor any to have the proposed document reviewed by both the hospice and hospital attorneys. An important lesson learned is to make provision for regular contract review and updating as circumstances and regulatory requirements may change over time.
