Post transitions between sites of care

The presentations today (9/17) did not go into this aspect of partnerships. What are the best ways to advise families and patients about these changed or changing relationships, which may involve new providers and changes in sites of care? At the United Hospital Fund we are considering a new family caregiver guide to add to our website (www.nextstepincare.org) about these transitions.
