I am in need of a sample agreement where in the hospital and Home care based hospice share the salary of a Nurse Practitioner to do Palliative care consults.

I don't have any agreement to offer. There is a possiblity that Hospice and Palliative Care Center of the Bluegrass in Lexington, KY and/or Hospice of Siouxland in Sioux City, Iowa may be helpful. Best wishes in your search.
