Post Electronic Medical Records questions

Greetings, all,
We are developing a partnership with our hospital and hospices to provide IP care for some of our patients. We use electronic medical records to document, get meds, charge, etc. Has anyone experience with the details of incorporating a contracted outside hospice into a setting with EMR's? We're getting stumped with issues of access, confidentiality, etc. Any ideas would be helpful! Thanks, Diane Ranna, NP
