Post Inpatient Hospice

We are a hospital-based Hospice program and are struggling with the process of switching an inpatient admission to an inpatient hospice admission. The need to switch these patients to hospice inpatient status has been occuring more frequently lately. Would anyone be willing to share their policies/procedures and/or process? Our current policy states that we continue with the same episode of care but we are now considering discharging the patient and readmitting to the same room (no change for patient/family, just a new account #).
