We do lots of hospital inpatient hospice transitions and are unable to do split-billing for one account number. Therefore, the patients are discharged from acute care and readmitted as an inpatient hospice patient. This keeps billing clear and assures that the plan of care has been thoroughly reviewed and the most appropriate plan is in place. We just received our first patient that had to be discharged and readmitted in this fashion and the process was less than smooth. Any assistance you can offer to make the process more seamless would be greatly appreciated.
