Post Hospital Palliative Care and Hospice Contract

We have been using a standard contract between our hospital, a 10 bed palliative care unit and several local hospices for about 10 years. We are in the process of updating it and would like to see any copies of recent contracts so we can incorportate any needed changes. We use two separate agreements, one for the services themselves and one to maintain HIPAA compliance, since both parties may serve as the covered entity or the business associate depending on the flow of information and since both entities create protected health information (as opposed to an insurance company which only recieves PHI) Any help is appreciated.
