When I attended CAPC training, I remember something being said in regard to the advantage to a hospital to not have a patient die in the hospital.

Our hospitals don't seem to be motivated to lower their mortality rate.

However, one of the hospitals with a palliative care program sends patients wihin hours of death to our care. Is that how it should work?

At my facility we have a hospice as part of our healthcare system with a liason in the hospital to see pt's. They have asked (with approval from the medical groups) that I follow pt's on palliative care who are imminent within hours to days. This is for a number of reasons. They state it is more cost effective for them. I am a nurse consultant and am an employee of the hospital, and do not bill. Also families are unhappy when their loved one dies in transport and usually prefer to stay here, besides feeling abandoned by the hospital and physicians when they are sent away...

Could someone please address the financial implications of different types of Hospital-Hospice relationships: 1) the two being independent of each other but with a Hospital/Hospice contract for beds; 2) the same arrangement but in an area where the Hospital is contracted with several hospices (and is a board member of a local not for profit hopice), some of the Hospices being for-profit. Also, the relationships when the Hospices are small and do palliative care for "free", since they do not have a valid billable service, yet do provide some palliative care.
