My hospice is associated with a hospital, what kind of information do I need to give them regarding a freestanding hospice house? We are non-profit.

There are several ways a freestanding hospice house can contribute to the hospital-hospice continuum of care. The services you offer in the freestanding faciilty make it an attractive location for patients who may be able to be discharged from the hospital but unable to go home for a variety of reasons. Does your hospital have a palliative care service? If so, it would be useful to meet with the key clinicians from that service to make sure they understand what kinds of patients would be well-suited to your site. You will also want to be sure that the hospital discharge planners are aware of your admissions criteria and service capabilties. Do you expect to limit admission to your house to hospice patients or would you consider palliative care patients as well?
