In the context of hospital-hospice partnerships, what can the two organizations do, structurally or otherwise, to work cooperatively and in tandem, assuring appropriate referrals in the community (minimizing confusion between the two groups) and smoothing the transition as a patient/family unit moves from one to the other along the continuum?

In our community the hospital and hospice have a collaborative program where the palliative care team is composed of persons from both organizations. There is also an outpatient palliative care clinic and a long term care program. As patient's needs change, the team communicates with the various other providers of palliative care to assure that palliative care is available across the continuum. This is the model that we talk about at the Palliative Care Leadership Center of the Bluegrass in Lexington KY.
