In our community the hospital and hospice have a collaborative program where the palliative care team is composed of persons from both organizations. There is also an outpatient palliative care clinic and a long term care program. As patient's needs change, the team communicates with the various other providers of palliative care to assure that palliative care is available across the continuum.
This is the model that we talk about at the Palliative Care Leadership Center of the Bluegrass in Lexington KY.
