Post Initial Consultation Assessment

We are creating a new consultative Palliative Care program at our facility. I am trying to find a good tool to use as the initial assessment of the patient which also includes the physical, psychological, social, cultural and spiritual aspects of the assessment. Does anyone have a tool that has worked well and that a plan of care can be developed as a result of the tool? Thanks.

Greetings--if you have not done so, I'd start by looking through the various patient assessment tools on he CAPC website,

The tools are free to download and adapt to meet your needs. If you do develop a new tool, please share a copy with CAPC so we can upload to the Tools section of the website.
