We are doing Palliative Medicine in a small hospital, as a consultation service. We would like to begin using automatic triggers (i.e. > than 5 days in ICU, frequent admissions, previous Palliative Care patients, etc.) in addition to the triggers given to us by the hospitalists. Would appreciate any information that you may have about this, especially if someone already has a policy about this in their program. Thank you!

We utilized PC triggers throughout St. John Providence Health System, Detroit, MI. We originally developed a front and back page screening tool and piloted it in a medical ICU, 2 med/surg units and oncology unit. After a year, we analyzed the data and determined the most frequent reasons for a PC consult and created a shorter trigger tool. Both tools can be found under CAPC's Tool section. The triggers have aided in increasing access to PC. I would suggest piloting the triggers in a few units to account for staffing capacity.
