I am curious why he can't go home on the dilaudid at 22mg/hr. The dilaudid can be concentrated to 10mg/ml so volume and need for cassette changes etc would be minimized.

Additional: Pt is having a lot of side effects: fatigue/confusion/muscle twitching. He is requiring a lot of breakthrough and does not want to go up on the the drip. We are looking for another option while he waits for Ketamine burst
