Post Methadone and prolonged QT/arrhythmias

We have a young gentlemen on a dilaudid drip of 22mg/hr. He has a strong desire to go home but his opiate requirements are too high. Methadone has been suggested in the past but he has experienced arrhythmias and has a QT of 480. Any suggestions would be helpful
