We have found that the most efficient hand-off is through the electronic progress note that our team uses. It includes the current assessment and plan for the care of the patient. Included in the documentation are the interventions, plans for disposition, family meetings held, etc. When we used paper documentation of the patient's symptoms that was a great tool because it was updated with each visit and we could see the progression of the care and symptom management. The other method we use that is tried and true but not structured, is a phone call to update our colleagues at beginning or end of day if they will be covering the care.
