Post Care Plan Format and Process

We are updating our care plan format and process. We are interested in benchmarking with other hospice and palliative care agencies to see what you do and what has worked best for you. Our goal is to have an interdisciplinary plan of care that meets regulatory and professional standards, is easy to understand and most importantly reflects the goals and wishes of the patient and family in a clear and concise manner.
