This is a very challenging topic that many palliative care programs are dealing with in the outpatient arena. There is no single consensus on management. As a start I would suggest reviewing some of the excellent articles by Steve Passick on this issue as well as search for the various tools available for screening for abuse potential in the outpatient clinic. Following this, the palliative care staff need to come to consensus on 1) their own needs for education in patient management, 2) a process for developing a team consensus on assessment / treatment decisions.
