Post Generalist/Primary Palliative Care

In January CAPC published a consensus paper on integrating triggers to identify patients at high risk for unmet palliative care needs on hospital admission or during daily rounds (http://www.capc.org/research-and-references-for-palliative-care/pivotal-palliative-care-articles)
). Note--this list was not a trigger list for PC specialist palliative care consultation, but rather for improving generalist palliative care services.

I am looking for hospitals/health systems that have started to work on this issue--please contact me if you have started, or are interested in starting, to work on this new approach to case finding.
