Since our program's inception,we have had weekly team meetings to discuss our consults, what each of our roles is, and the general plan for the patient. "Minutes" are taken, and stored in a binder, and also electronically. They have not been part of the medical record, although they do have patient names, and particular clinical information. They are only accessible to the core team members. Are other teams doing this as well? What kind of documentation do you keep of your team meetings.

The teams at our hospitals and others I have worked with do not keep written notes of the discussions of patients. (We do keep notes about discussions at staff meetings.) It seems important to discuss with your team the purpose of keeping these notes. Is it for collecting operational data, measuring clinical impact, etc? Are there other sources of this data? Another question is whether the information you record should be included in the medical record to be accessible to other members of the health care team. If the information is also entered in the medical record, would that meet your needs, avoid duplicate charting and minimize risks to patient confidentiality?
