Since our program's inception,we have had weekly team meetings to discuss our consults, what each of our roles is, and the general plan for the patient. "Minutes" are taken, and stored in a binder, and also electronically. They have not been part of the medical record, although they do have patient names, and particular clinical information. They are only accessible to the core team members. Are other teams doing this as well? What kind of documentation do you keep of your team meetings.
