Does anyone have a standardized comfort care narcotic drip MD order form that includes additional prn meds such as Ativan?

Hi,
I hope some others may reply with examples of order sets from their institutions, but CAPC has assembled a nice collections of clinical tools and orders sets that you can review and use.
